Tax Meeting Preparation List

Use this list to gather your documents and information for your tax preparation appointment.

  • Prior year's tax return - new client's only. (If we prepared your last return we have it on file.)
  • For all persons in your household: full name, birthdate, and Social Security number.
  • All W-2 forms. These show your income for the year of the return.
  • 1099R forms. Retirement income: pensions, IRA distributions, Social Security benefits paid, and more.
  • 1099-INT forms. Interest income from all sources.
  • 1099-DIV forms. Dividend income from all sources.
  • 1099B forms. These document sales or exchanges of assets, such as, mutual funds, stocks, and bonds.
  • 1099G form. Unemployment benefits received.
  • Miscellaneous Income - alimony, spousal support, jury duty, gambling winnings, education savings account withdrawals, and other income.
  • Self-employment income and expenses.
  • Rent and royalty income and expenses.
  • Education expenses.
  • Itemized deductions. Include: medical expenses, real estate and sales taxes paid, mortgage interest paid (form 1098), charitable donations (including both cash and non-cash donations), unreimbursed employee business expenses, job seeking expenses for seeking employment in the same field, tax preparation fees, investment expenses, and gambling losses (limited to the extent of winnings).

Charitable Donations

A deduction for charitable donations of non-cash items is typically the fair market value (FMV) of the goods donated. It is the taxpayers responsibility to document the FMV of each item donated. Click here for links to donation valuation guides.

Documenting Non-Cash Charitable Donations

Taxpayers written records documenting charitable donations must include:

  • Name and address of the organization to which items were donated
  • Date and location of the contribution
  • Description of the property
  • Fair market value of the property
  • Cost basis of the property
